Description | Please direct all calls to Where to Start Access to Mental Health Services *905-451-4655 The Peel Wraparound Process is a community-based outreach service offered to residents of the Region of Peel. The inherent case management approach assumes that clients with complex and multiple needs will access services from a range of service providers with the goal to achieve seamless service delivery. Wraparound provides individualized case management as a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s holistic needs through communication and available resources to promote quality outcomes. It is a conceptual framework with which improvement in health and well-being are achieved by directing efforts towards addressing the social and environmental factors of health, concurrently with biological and medical factors. It focuses on the development and maintenance of a system by implementing the Wraparound Process to create and sustain structures to support children, youth and their families with complex needs who have not had success with traditional services. Wraparound is a team based planning process intended to provide individualized, coordinated, family-centred care to meet the complex needs of children and youth who are involved with child/youth/family serving systems (e.g. mental health, child welfare, juvenile justice, special education) who experience emotional, social, behavioural, or mental health challenges. |

Eligibility | Ages: 18 year(s) and under Wraparound Process would have the following characteristics: *child/youth is at risk of harm to self or others *child/youth is at risk of experiencing family breakdown or losing his/her placement in home, school and/or childcare setting; however, there is motivation by the family and/or child to work on presenting issues *child/youth has witnessed violence, suffered physical abuse, and may have additional medical/ developmental delays *families are overburdened by multiple stressors and have diminished coping capacity and problem solving-abilities. These stresses may be acute or long-standing, contributing to isolation in the community. • a history of mental health issues for the parent and/or the child/youth. Mental health issues may be compounded by multiple service involvement with other systems, or through living in an environment where there may be violence, alcohol/substance use, caregiver mental health issues and/or sexual abuse. Previous services have been unsuccessful in improving the situation. *children or youth are at risk of requiring services legislated by the Youth Criminal Justice Act. Peel Wraparound is available to children and youth who require outreach to their home, school or community, and who have been unable to make use of, or benefit from, traditional agency services. |
