Description Access to health care support at home, at school or in the community * care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * care is provided in community settings such as the home, school, residential facility or nursing community clinic * provides information about other community services and refers when appropriate 
 
Services available to eligible patients may include: 
Nursing 
Nurse practitioner 
Personal support 
Physiotherapy and occupational therapy 
Speech and language therapy 
Social work 
Nutrition and food counseling (dietitians) 
Palliative care 
Medical supplies and equipment 
Access to long-term care homes 
Access to supported living (adult day programs, supportive housing, retirement homes and assisted living) 
Health support in the school 
Specialized services for people living with specific health conditions 
Referral to other community supports 
 
Information and Referral - thehealthline.ca provides online access to health care information and services in Ontario, or call the referral phone service at 310-2222 - in English or 310-2272 - in French  
 
Children's Health Services - Special health services for children in both the home and school environment * supports children when health care provision is essential to a student’s participation and development in school programs * services are provided within public, private or home-based schools 
 
Health Links - Designed to help high-needs patients such as seniors and people with multiple health conditions, stay safe at home and out of hospital and emergency rooms * provides an enhanced level of care through improved coordination of care and communication between health care providers, including primary care, hospitals, long-term care, mental health and addictions and community support services * there are 7 Health Links in Mississauga Halton area 
 
Health Care Connect - Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by the LHIN - Home and Community Care * Care Connectors work with family doctors and nurse practitioners who are accepting new patients 
 
In-Home Services - Health care services to people in their own homes 
 
Mental Health and Addictions Nurses - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate 
 
Placement Services - Determine eligibility for admission to long-term care facilities and prioritize and manage the admission process * support and information is provided to applicants and their families prior to and during the admission process 
 
Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community 
 
Supported Living - Information and/or assessment for Adult Day Programs (ADS), Retirement Homes, Supports for Daily Living or Supportive Housing

Eligibility People of all ages who are facing the challenges of age, illness, injury, or disability
Application Residents who have a valid Ontario Health Card are eligible to apply for in-home services (home care) and for admission to long term care facilities
The Ministry of Health and Long-Term Care has set criteria for eligibility
Eligibility is determined by the LHIN - Home and Community Care Coordinator
Fees None ; Funded by the Ontario Ministry of Health and Long Term Care
Languages English ; French ; For other languages, interpretation services available
